I consent to the above participant (hereinafter "the Participant") taking part in Churakids Summer Camp 2026 (hereinafter "the Camp"). I confirm that the Participant's health condition is suitable for participation in the Camp.
The Camp includes outdoor activities such as snorkeling, kayaking, and campfires. I understand that these activities carry inherent risks, and I agree to release the organizer from liability for accidents or injuries that may occur despite the organizer's best efforts to ensure safety, unless caused by the organizer's gross negligence.
I will accurately disclose all relevant health information about the Participant (allergies, pre-existing conditions, medications, and other matters requiring attention). The organizer shall not be held liable for any accidents resulting from inaccurate or incomplete disclosure.
In the event of injury or illness during the Camp, I consent to the organizer providing appropriate first aid and arranging transportation to a medical facility as deemed necessary. Any resulting medical expenses shall be borne by the guardian.
The Camp provides accident insurance coverage for all participants. The organizer shall not be liable for damages exceeding the scope of insurance coverage.
I will ensure that the Participant follows Camp rules and staff instructions. I acknowledge that the organizer may terminate the Participant's involvement if they significantly violate Camp rules. No refund will be provided in such cases.
I consent to the use of photographs and videos taken of the Participant during the Camp for Churakids' promotional activities (website, social media, brochures, etc.). If you do not consent, please note this in the comments field and we will accommodate your request.
Personal information provided will be used solely for Camp operations, communications, and safety management. Disclosure to third parties is limited to the minimum necessary scope, such as insurance procedures and emergency medical information. Sensitive data, including health information, will be properly disposed of after the Camp concludes.
* "Event date" refers to the first day (gathering day) of each session.
* Cancellation fees are calculated based on the actual amount paid (tax included) after discounts.

No refunds will be provided in the following cases:
- Absence on the day due to illness or early departure during the Camp period
- Partial program cancellation due to weather conditions during the Camp period
Cancellation due to natural disaster: If the Camp is cancelled before the event due to a typhoon or other natural disaster, a refund will be issued minus a $35 preparation fee.
Refund method: Refunds will be made to the credit card used for payment. If a bank transfer is required, the transfer fee will be borne by the customer.
Date change: Accepted only if space is available. Any price difference will be settled accordingly.
